I've had my Yellows rebuilt every 40k miles. That's probably overkill, but I use them on the race track from time to time. I've heard 80k rebuilds are a good idea.

Incredibly common on the e46. I have replaced a ton of them. I would get an oem piece used from a retailer and re-wrap in shadowline tape. If not, prepare to be bled George Washington style by BMWNA....

I would be incredibly suspect of a failed steering rack. What evidence did they give you? I have customers with over 300k miles on e46s and their steering racks are still holding up properly.
I could theorize a few different causes, mostly related to temperature/ improper installation. Ultimately, I hate it happened, but I'm thankful it was just the CCV and not something more...
